Overview

The purpose of this project is to digitize laboratory test results into a structured spreadsheet for internal reporting and record management. The spreadsheet must be accurate, well-organized, and handled with strict confidentiality due to the sensitive nature of the data.

Scope of Work

The task involves:

• Reviewing lab test documents provided by the client

• Accurately extracting test and patient-related information

• Entering data into a structured and clearly labeled spreadsheet

• Verifying entries for correctness and completeness

• Ensuring data confidentiality throughout the process

Required Fields

The spreadsheet should include:

• Patient ID

• Test Name

• Date of Test

• Test Result

• Reference Range

• Lab Technician Name

Additional fields may be added if required by the client.

Client Material Source

All information will be provided by the client from official laboratory reports.
No external research or interpretation of results is required.

Deliverables

• One completed Laboratory Test Results Spreadsheet

• Format: Excel or Google Sheets, as specified by the client

• Spreadsheet must be clean, well-formatted, and ready for internal use

Quality & Confidentiality Requirements

• High level of accuracy in data entry

• Correct date and numeric formatting

• Strict confidentiality and secure handling of all records

• No sharing, copying, or external storage of client data

Deadline

The final spreadsheet must be delivered on or before 4th February 2026.
